CASE MANAGER (MAA/TCM)

BGCGG provides hope and opportunity through school age, family strengthening, and community outreach programs, offering over 60 program locations that serve over 6,000 children daily and 10,000 families annually. Class:                          Non-Exempt, Full TimeHourly Rate: Starting at $14/hour + comprehensive benefits

Hours:                        30+ hours/week, occasional evening and Saturday hours

Primary Function: Helps the programs interact with the community both in the office setting and at community events. Assists with the development of ARCHES programs and integration of services with Club sites.

 

Health & Safety

·       Ensures the ARCHES office meets health and safety standards and ensures the safety of all clients [1 or 21]

·       Takes actions to prevent accidents and responds to injuries and signs of illness [1 or 21]

 

Program Development & Implementation

·       Effectively supports the planning, development and implementation of ARCHES programs, services and activities [1 or 21]

·       Effectively monitors and evaluates programs, services and activities through tracking referrals, phone calls, customer service and any additional responsibilities; assists supervisor with training and supervision of volunteers [1 or 21]

·       Ensures a productive work environment by working as a team member, contributing to team meetings and providing feedback to support decision making; continuously manages own time and resources [1 or 21]

·       Continuously develops own knowledge and skills in each of the various Family Strengthening programs in order to diversify skills [1 or 21]

 

Technology

·       Effectively uses Microsoft Office applications, electronic scheduling and database software [1, 3 ,5, 4, 6, 21, 23, 24, 25, 28]

·       Completes daily process notes for each client worked with and enter information into database [1, 3, 4, 5, 6, 21, 23, 24, 25, 28]

·       Documents all services rendered including phone contact, completed services and records in database [1, 3, 4, 5, 6, 21,  23, 24, 25, 28]

 

Partnership Development

·       Builds partnerships with parents, families and community partners [1, 3, 4,  5, 6, 7, 8, 21, 23, 24, 25]

·       Attends coordination meetings, case management and informational meetings in order to maintain and develop community partnerships and other meetings as necessary [1, 3, 4, 7, 8, 20,  21, 23, 24, 25]

·       Assists with special events, makes necessary follow up calls, reports all child abuse cases to CAR and completes reports [1, 21, 25]

·       Complete agency approved case management training; assists county WRAP families with case management [1, 3, 4, 5, 6, 21, 25]

 

Medi-Cal Related Responsibilities

·       Attends trainings regarding the Medi-Cal screening process, including updates regarding changing requirements [8]

·       Assists families in re-applying for Medi-Cal, if applicable, and discusses follow-up health care [4, 8]

·       Conducts follow-up phone calls to participating families to verify Medi-Cal recertification, and access to Medi-Cal covered services for needed health care [6, 8]

·       Assists children and families in accessing/utilizing medical services, including Medi-Cal covered health services, while encouraging prevention and awareness [4, 24, 25]

·       Assists the Program Supervisor in providing basic preventative health care information to children birth to 5 years of age, and their families in the community, including information on health and Medi-Cal services program resources [ 3, 4, 24]

·       Acts as a patient advocate for clients requiring assistance with healthcare/provider issues for the purpose of facilitating access to health insurance coverage including Medi-Cal [4, 6, 24, 25]

·       As a Certified Application Assistor, provides information to and screens potentially Medi-Cal eligible children and family members to obtaining health care coverage, including Medi-Cal [8]

·       Actively encourages parents regarding enrolling uninsured children in health insurance, including Medi-Cal/Healthy Families, thereby meeting standards for application completion and lead generation (referrals) [8, 24, 25]

·       Advocates for families enrolled in Medi-Cal needing health care services by making appointments, assisting them in utilizing Medi-Cal covered health care services; and providing follow-up with families as needed [4, 6, 24, 25]

·       Assesses and evaluates the needs of families. Provides appropriate initial referral assistance to patients for health care providers, health services and community-based agencies, based on assessment findings [4, 24, 25]

·       Tracks and follows up resource referrals on previously assisted Medi-Cal eligible children and family members, including related to their access to health services [5, 6, 24, 25]

·       Presents to outside agencies to increase referrals for health and Medi-Cal services [4]

·       Assists in outreach activities at health fairs, community events and other [3, 4]

·       Arranges transportation for clients to access health services [10, 24, 25]

·       Acts as English/Spanish translator when interpretation is needed for health services and activities related to Medi-Cal Outreach and facilitating Medi-Cal applications [4, 6, 8, 24]

·       Increases linkages to health services through intra and inter-agency outreach [4]

 

Communication

·       Maintains close, daily contact with staff, members and supervisors to receive and provide information, discuss issues, explain or interpret guidelines and instructions [1, 3, 4, 21]

·       Maintains contact as needed with external community groups, schools, members, BGCGG partners and others to promote and ensure information and referral services for the Family Strengthening Programs are successful [1, 3, 4, 21, 25]

 

Qualifications

·       Must be bilingual in Spanish and at least 21 years of age

·       Four years relevant experience OR AA degree and 2 years relevant experience OR Bachelor’s degree and 1 year case management experience

·       Experience working with families and youth; Medi-Cal experience preferred

·       Reliable transportation, safe driving record, driver license, 2 years of driving experience and automobile insurance required

 

Knowledge, Skills, & Abilities

·       Demonstrated experience in case management and knowledge in case management philosophy and function, enforcing the mission

·       Ability to work with diverse populations in a culturally competent manner

·       Ability to be resourceful and possess a knowledge of community based  mental health services

·       Strong organizational skills with attention to detail, ability to follow-up and manage demanding caseload

·       Ability to prioritize and complete multiple tasks within expected deadlines; ability to adapt and be flexible

·       Strong communication skills; both written and verbal, and ability to provide feedback regularly

·       Excellent documentation skills with demonstrated ability to write clearly, concisely and descriptively

·       Knowledge of HIPAA and confidentiality regulations; ability to follow organizational procedures

·       Ability to contribute to the quality of a high performing team; ability to take on different roles as needed

·       Skills working one-on-one with youth; ability to speak in front of large and small groups

·       Proficiency in using a variety of computer software applications, specifically, Excel, Word, Publisher and Outlook

·       Skills in project management; skills in providing training; networking skills and ability to establish collaborative relationships

·       Knowledge of administrative and clerical procedures and systems such as managing files and records, following up on phone calls, receiving and documenting payments, conducting meetings, completing reports, and tracking data

 

Requirements

·       Position may require standing/sitting for extended periods of time, dexterity of hands and fingers to operate office equipment, reaching overhead, above the shoulders and horizontally, bending at the waist; seeing to review and analyze reports, documents or other reports; hearing and speaking to communicate and provide information to others; speaking to be heard before groups of people; position may require to lift, carry, pull or otherwise move objects between 10-20 pounds
